Planning for Your Future: Fraser Health Services for Older Adults

Join us for an information session with experts from various programs at Fraser Health Authority exploring services offered to older adults. This is important information for you as you plan for aging in place or for adult children looking to support aging parents.

Each program from Fraser Health will give a 20 minute talk and then participants will have the opportunity to ask further questions and get more information at each program booth.

Advance Care Planning

Learn about the 5 steps of Advance Care Planning, how to choose a substitute decision maker and how to share and document your wishes for future care.

Falls Prevention

Discuss the 4 pillars of falls prevention (eyesight, medication, exercise, home safety); what you can do to reduce your risk of having a fall.

Home Health & Social Prescribing

Learn about the eligibility criteria and how to access services provided by Community Health teams in your area. Learn where to find resources available in your community, such as social prescribing, BC211, and websites that share healthy aging information.
